Insurance & Billing

Perennial Mental Health, LLC is out-of-network from insurance. However, we do accept HSA’s/FSA’s, and are happy to provide a superbill that you can submit to your insurance provider for reimbursement. 

We charge a reasonable rate for individual and couple’s/family sessions, and offer a generous sliding scale if needed. We are willing to work with you to find a rate that works best for your life and finances.

Helpful terms to know:

  • HSA (Health Savings Account) and FSA (Flexible Spending Account) -
    Employer-sponsored accounts used to pay for various medical expenses. Basically a bank account that is solely used for copays, therapy costs, and other health-related expenses.

  • Superbill -
    An itemized, comprehensive "receipt" provided by an out-of-network provider to the patient, who can then submit it to their insurance company to seek reimbursement for the cost they paid for those services. Superbills are usually provided monthly.

  • Sliding-scale -
    A pricing model that adjusts the cost of services based on one’s financial circumstances or income, and allows for an agreed-upon price that is lower than the standard rate. The sliding-scale model is designed to make services more accessible to everyone.
